The north-south distance from the Pennsylvania state line to the West Virginia state line is only 1.8 miles (2.9 km) at Hancock.

According to the United States Enumeration Bureau, the town has a total region of 2.75 square miles (7.12 km2), of which, 2.73 square miles (7.07 km2) is territory and 0.02 square miles (0.05 km2) is water. The state of Maryland narrows to a width of less than two miles in the Hancock region the smallest non-vertex border-to-border distance of any U.S.

The Chesapeake & Ohio Canal passes through the town, and the Western Maryland Rail Trail joins Hancock with Fort Frederick State Park.
